ALS Patient Who Lost His Voice Uses Neuralink to Tell His Wife ‘I Love You’: ‘A Beautiful Surprise’

Kenneth Shock thought of three words he had increasingly struggled to say as Amyotrophic Lateral Sclerosis (ALS) affected his speech: 'I love you.' The Neuralink system then produced them aloud for his wife, Cheryl, using a reconstructed version of the voice the disease had gradually taken from him.

The words did not come from his mouth. Neural activity associated with his intended speech was recorded by his Neuralink brain implant and decoded into spoken output.

Neuralink shared the 23-second clip on 16 September with a simple caption: 'A beautiful surprise.' Elon Musk later shared the video with an equally brief reaction: 'I love you.'

The moment itself was not a newly conducted September experiment. Neuralink had previously documented Kenneth and Cheryl's voice-restoration work in March, when Cheryl described hearing him tell her he loved her through the system.

For Kenneth and Cheryl, however, the significance went beyond demonstrating an experimental brain-computer interface. It meant hearing something familiar again.

Neuralink Clip Draws Emotional Reactions

Shock, who has ALS, received Neuralink's N1 implant in January 2026 at UT Southwestern Medical Center in Dallas. Neuralink has described him as the second participant to receive an implant for its speech-restoration work.

Neuralink had introduced Kenneth and Cheryl's story earlier in the year, documenting how ALS progressively affected his movement and speech. Cheryl said Kenneth's voice was one of the things that first attracted her to him after the couple met online.

The newly isolated clip put one of their most personal moments centre stage.

Neuralink co-founder, president and COO DJ Seo reacted on X by writing, 'I'm thinking "I love you".' He later provided more detail about how the system produced the words.

How Does the Neuralink Brain Implant Turn Thoughts Into Speech?

Despite its futuristic appearance, the N1 does not simply read whatever someone happens to be thinking.

Neuralink's brain-computer interface records neural activity associated with a participant's intention to speak. Normally, regions involved in speech generate signals that ultimately help control movements of the mouth, tongue and voice box.

ALS progressively damages motor neurons. As the disease advances, signals involved in voluntary movement may no longer produce normal muscle movement or intelligible speech.

A brain-computer interface can attempt to bypass part of that impaired output pathway. It records relevant neural activity and uses software to identify patterns associated with intended speech.

The N1 is a skull-mounted, wireless and rechargeable implant. Neuralink says its system records brain activity through 1,024 electrodes distributed across 64 flexible threads. The R1 robot is used to place the electrode threads in the brain.

Software then analyses those neural patterns and attempts to decode the participant's intended speech into text or spoken output.

Seo provided an important detail after the September clip circulated. He said Kenneth initiates the process with a key press, thinks the words 'loudly' and then commits the output with another key press.

That makes the demonstration different from unrestricted 'mind reading'. The participant deliberately starts and confirms the communication process.

The voice is also significant. Seo said the output uses Kenneth's own voice and was trained and generated with Grok. Neuralink's earlier material said the goal was to reproduce his voice from around 2020, before ALS significantly changed how he sounded.

What Is Neuralink's VOICE Trial?

VOICE is an early feasibility study examining the initial safety and effectiveness of Neuralink's N1 implant and R1 system as a way to help people with severe and irreversible speech impairment communicate.

The study is registered as NCT07224256. It began on 3 October 2025 and is recruiting through UT Southwestern Medical Center in Dallas. Estimated enrolment is six participants.

Potential participants include people with ALS, primary lateral sclerosis, stroke or spinal-cord injury who have severe speech impairment and impaired upper-limb function.

Safety remains a major focus. The study's primary outcomes examine device-related and procedure-related adverse events at 12 months. Preliminary effectiveness of the N1 implant is among the secondary outcomes being evaluated for up to 48 months.

The target of only six participants means Kenneth's demonstration cannot establish how well the technology would work across the wider population.

Neuralink says its devices remain investigational and are not commercially available or FDA-approved.

The company received FDA Breakthrough Device Designation for its speech-restoration technology on 1 May 2025. The programme can increase interaction with the FDA and expedite development and regulatory review, but Breakthrough designation is not the same as FDA marketing authorisation.

For now, the N1 remains an experimental implant being evaluated in clinical trials.

The latest clip puts the human side of Neuralink's research into clearer focus. Beyond electrode counts and decoding technology, the goal is simple: helping someone whose ALS has severely affected his speech tell his wife, 'I love you,' in a voice made to sound like his own.
